WHO, WHY & WHEN When are Work Permits undertaken? • When conducting non-routine work activities • When conducting routine work activities which are subject to specific restrictions Work Permits are undertaken PRIOR TO the works commencing. Why are Work Permits undertaken? • To establish if the proposed work activity can be safely undertaken in an acceptable manner, as far as practicable. Who can undertake a Work Permit? • Manager / Supervisor ONLY • (HSEQ Unit available to assist if required) 12/07/2006 Rev B

  5. A JSA APPROACH When performing a JSA (SWMS) of a work process for a work permit pay attention to these concepts. • Concentrate on what has to be done • Do not concentrate on how job is to be done. • Look at each step and identify all the hazards. 12/07/2006 Rev B

ASSESSMENT • An assessment of the proposed work activity may typically involve: • Undertaking an inspection of proposed work area • Having a good understanding of work permit location (products stored, workplace activities occurring, underground wiring, fire protection system configurations). • The Work Permit must be undertaken in consultation with the Permit Holder. • Once initiated, the Work Permit is registered in the site’s Work Permit Register. • The Work Permit requires sign off by the Manager / Supervisor AND Permit Holder, both prior to and upon completion of the works. Site Work Permit Register can then be closed off. NOTE: One of the most important things to remember is to keep an open mind about what can go wrong. 12/07/2006 Rev B

  8. ASSESSMENT Cont. How to complete a Work Permit assessment? • Obtain copy of Work Permit Form (F-130) • Complete relevant details of Permit Holder (Contractor name & Business name) • Register Work Permit on Site Work Permit Register. 12/07/2006 Rev B

  9. ASSESSMENT Cont. In consultation with and in presence of the Permit Holder, determine: • The Work Permit Activity • The individual Tasks required to perform the activity in the sequence that they are carried out. • The Hazards of each task that could cause injury/incident • The Risk Control Measures required to eliminate/ minimise the risk arising from the hazard. • The Person responsible (Permit Holder or Mgr/Spvsr) to implement risk control measures. 12/07/2006 Rev B

Discuss and determine with Permit Holder: • Type of equipment expected to be used • PPE requirements • Location, Date and Time works are authorised for • Whether Safety Attendant /Guard is required • Whether any equipment requires Tag Out • Whether vehicles are required to be brought on site 12/07/2006 Rev B

  11. ASSESSMENT Cont. Discuss and determine with Permit Holder: • Whether the proposed work area is a Hazardous Area. If yes, additional considerations for hazard identification and control measures may be required. If the proposed work location is an area where explosive atmospheres may be present, hand tools rather than electric tools need to be used. • Whether there are any other related Work Permits underway / scheduled that may impact on these proposed works. 12/07/2006 Rev B

Section 3 & 4 Of Work Permit Form • Upon completion of works, Manager/ Supervisor to inspect works for satisfactory completion and ensure work permit location has been made safe PRIOR to signing declaration. • Should the works be deemed incomplete or unsatisfactory, Section 4 is to be addressed by the Supervisor/ Manager. 12/07/2006 Rev B
