Building Aging at Home • Invest $21 M in new funding over next 3 years … establishing the local Directional Plan • Nine Focus Groups (talking to diverse groups of seniors) • Seven Public Forums (one in French) • Aging At Home Symposium (Paul Williams, Mark Nowaczynski - innovation and integration) … and the local Detailed Plan • Three Public Validation Forums • Over 1000 people engaged • Services for Seniors Core Action Group

  5. Directional Plan • Increase capacity to relieve pressures • Address priorities identified through community engagement • Support target population (at risk of institutionalization) • Put in context of IHSP and Ministry priorities • Monitor and evaluate impact

  6. Year 1 Service Clusters • Congregate Dining • Adult Day Programs • Home Maintenance / Home-Making • Seniors multi-resource centre • Day program for seniors and care-giver • Volunteer training centre • Supportive Housing • Transportation • Tele-chek

Year 1 Service Clusters Specialty Programs • Nurse Practitioners in LTC facilities • “Home-at-Last” across LHIN • Physician home-visiting • Social, recreational, and health educational programs • Seniors helping Seniors Wellness / Equity • South Asian wellness, volunteer program • South Asian Adult Day Service • Expand Ethno-cultural Seniors Centre

  8. Year 1 - Learnings • We’re on a learning curve • Still lots to do • Look at Year 1 processes • Review initiatives funded by all LHINs • Look at Year 1 initiatives moving forward

Central West LHIN Year 2 Aging at Home • Determine gaps (geography and services) after Year 1 • Targeted Year 2 investments • Invite “geo-clusters” to develop Year 2 initiatives • Business cases received, reviewed, and categorized into “direct / immediate” impact on ER/ALC, “diversion” away from ER/ALC use, or “prevent” ER/ALC use • We’re setting the stage for Year 3

Year 2 – Learnings, so far • Lots of diversion and prevention initiatives • Need for unbundling • Need to finalize costs and service levels • Difficult to fund new start, high-cost proposals, notably • supportive housing • LHIN needs to work with group of providers to develop “direct / immediate” initiatives • Always keep an eye on Integrated Health Services Plan and Health System Plan • It’s about seniors and their health!
