Payroll Processing Overview • In order for payrolls to be processed prior to the transition: the payroll documentation, funding, and supporting documentation must be submitted to SBA by 4:00pm ET on January 10, 2014 • Note: Payrolls after this cutoff should be held onto and processed through the Plan Service Center website (PSC) during the first cash appointment • The PSC Authorization and Payroll Data Survey forms must be completed and sent in before December 13th. Please makes sure you send back both pages • Individuals will be assigned user id’s to access the PSC based on the information completed in the Payroll Guide. These id’s will be emailed on January 13th, 2014

Payroll Processing Overview: Payroll File • Great West will be setting up the system to read the file layout that we have received from the previous record keeper • There is no need to send us a copy of what you have been sending. We will make adjustments during the first cash call, if necessary • The file will need to be saved as a .csv • Payroll file should have the following information: • Payroll Location (Your employer’s existing 4-digit numerical code) • SSN# - with dashes • Deferral Amount - in dollars • Money Type – EE or ER • Pay Frequency - M • Last Name • First Name • Middle Initial • Percent/Dollar – field will be ignored • Bill Date • Reason For Change – please use this field to provide a termination date only. No text or notes, just a date. Termination is defined as participants that are no longer employed at your location

Payroll Funding Information • ACH funding: • Your financial institution will need to authorize Great West to draw the ACH for funding of contributions • If you have an ACH debit filter on the bank account used to fund payroll transmissions, you will need to provide the financial institution with the Company Name and ACH ID. • Company name: STATE OF MS • ACH ID: 2941347393 • The amount of time it takes to make this change can vary by bank. We recommend the plan contact their bank at least 10 business days prior to the plan submitting their first payroll contribution to us

  6. Payroll Processing: First Cash Calls First Cash appointments cannot take place prior to the 1/24/2014 This appointment will be scheduled with your Payroll contact. They will discuss a couple of times and dates with you We will verify your ACH banking instructions during this call to make sure we have the correct routing and account set up

  8. Participation Agreement Form-Deferral Report Each Employer will receive a Deferral Report that will explain the new dollar amount that the employee has elected. You will be notified via email that this report is available. This report will be sent first business day of the month This report can be retrieved from the Reports menu on the Plan Service Center under the Manage My Reports section
