  3. Criteria for Consideration • Non-profit or governmental status • Cost-free services • Discount services • Local affiliates • Compliance with Title VII (http://www.eeoc.gov/policy/vii.html)

  4. Community Constituents • Social Services • Educational Institutions • Health Institutions • Religious Institutions • Entertainment Venues • Community Centers • Local Chapters of National Organizations • General Public • Senior Citizens

  5. Service Provider’s Webpage • Contact information • Hours of operation/availability • Paperwork or forms needed to qualify • Link to individual social service’s homepage • Link to join organization’s mailing list

Provider’s Tools • Secured provider access forum • Requires Login ID and password • Access to all information, but can edit only their own information • Follow ethical codes of conduct appropriate to profession (e.g. HIPAA) • Link provided at the Hoganville Social Services Directory Website • Staff directory of other service providers

Functionality and Control • Website to be developed, hosted and maintained by Six Peeps Services for initial year • Collaborative partnership with Hoganville Public Library to be established for continued maintenance • Library will assume responsibility for year 2 onward • A relational database stored on a single server with a secure web interface allowing internet connectivity • Yearly report automatically generated for review by participating organizations for update purposes • Individual organization will update own contact information

  14. Implementation • Homepage established • Service providers submit and receive approval of webpage • Homepage goes live • Classes held at various locations to introduce homepage to general public • Additional webpages added as needed • Basic acceptance and stress testing

  15. Evaluation • Counter will record number of visits • Anonymous surveys will be randomly offered • Webpage will be added by providers to “referral information” on check-in sheets • Periodic review with community service providers will be held with town advisory board
