  6. MU Measure Type of Measure: Rate Patient Reminders

  7. Threshold More than 20% of all unique patients 65 years old or older or 5 years old or younger were sent an appropriate reminder during the EHR reporting period. Patient Reminders

RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: an active health record at the beginning of the EHR reporting period AND HAVING: no date of death recorded at the beginning of the EHR reporting period HAVING: age of 5 years old and younger at the beginning of the EHR reporting period OR HAVING: age of 65 years old and older at the beginning of the EHR reporting period Patient Reminders

  9. RPMS Reporting Logic Numerator Inclusions: COUNT: Count each patient in the denominator HAVING: one or more Patient Wellness Handouts generated during the EHR reporting period Patient Reminders

  10. RPMS Reporting Logic Measure Exclusion: • The EP is excluded from this measure if the facility does not have any patients in the database who are: • 5 years old or younger • 65 years old or older Patient Reminders
