Comprises of 59 acute beds (including dialysis, day procedure and midwifery) and 82 aged residential care beds. Services are provided to outreach locations including Sea Lake, Manangatang, Balranald, Kerang, Cohuna & Tooleybuc (all within 100-130km radius of Swan Hill). Support has also been provided to Sea Lake and District Health Service & Manangatang Hospital.     3. The organisational chart depicts the 3 separate divisions of Corporate, Clinical and Primary Care services. These divisions are overseen by our CEO and Board of Directors. Health Promotion Department Staff – 2 EFT Health Promotion Officers - 1 EFT Community Health Nurse - 0.7 EFT Head of Department GOAL The Health Promotion Department will provide a co-ordinated approach to the provision and delivery of a range of services and programs that addresses the health needs of the Swan Hill community. The programs and services will reflect National and State initiatives and address local priorities. In August 1990 the Swan Hill and District Community Health Service was formed, consisting of Alcohol & other Drug Service and a community health service. This was disbanded in July 1996 and both services became part of the Primary Care Division of the Swan Hill District Hospital. The Community Health services was renamed the Health Promotion Department soon after the amalgamation. Today the HP department provides a diverse range of programs and services including -health education to schools(pre to secondary) -community health education & promotion -screening & education services/clinics such as well women, diabetes, heart health -information & resource provision Coordination and support for self-help groups -promotion of health & hospital services (including ‘My Hospital’) -Collaboration and partnership on community health issues (PCP, grants, etc) -Workplace health promotion. Staffing has grown from the original part-time CHN and CDW to 2 EFT HPOs, 2 CHN sharing 1EFT overseen by myself (approximately.7EFT) also managing Dietetics and other projects. Health Promotion Business Plan Originally developed to outline Health Promotion Department Activities/Services Developed hospital wide Health Promotion Plan 2001 to include all activities of Primary Care Division Expanded 2002 plan to include Health Promotion Activities provided by Clinical Departments Used to formulate the DHS-Community Health requirement for Planned Health Promotion Activities FUTURE- Integrated Health Promotion Activities to be part of Business & Service Plan of each Department Based on 13 State Wide Health priority areas, and local services needs and gaps. Swan Hill District Hospital A Health Promoting Hospital AIM To incorporate health promotion concepts, standards and values into the organisational structure and culture of the Swan Hill District Hospital to improve the health and wellbeing of patients, staff and the broader community. Operates under the Occupational Health & Safety Committee. Three specific target groups – patients, staff and the community. Primarily this year the focus has been on staff. Aim incorporates parts of the WHO’s definition of HPH The HPH project has been on the agenda over the past few years, but only recently have we given it more structure and direction. Traditionally the OH&S committee, as I’m sure most committees operate, has had a definite safety focus – that’s where the big costs to an organisation are. We have progressed though, to an inclusion of staff health into the OH&S agenda. This has also been done in an attempt to include other areas of the hospital in health promotion. When I mention the term patients, I’m doing so in a generic term. Throughout the hospital we also have what we term clients (for example at 13 Pritchard St and Allied Health), we have women in midwifery etc. One difference is that generally when a HPH is discussed, it’s target groups are staff, patients, family and friends, so relates to the direct hospital community.     7. Staff Health Promotion Aim To provide a range of holistic health services for staff at Swan Hill District Hospital to improve or maintain positive health and wellbeing. Linked to Workplace Health Promotion Holistic approach – acknowledge that there are a broad range of factors including social, economic, cultural, environmental and organisation factors which significantly affect an individuals health status. Utilise the Ottawa Charter for Health Promotion (1986) and its five key action areas – Formulating Healthy Public Policy - Creating Supportive Environments - Strengthen Community Action - Develop Personal Skills - Reorient Health Services To provide some structure for our activities they have been grouped under the 5 key action areas of the Ottawa Charter for Health Promotion. Why we decided to target staff with WPHP, what we do in our community role, we try to do in the hospital as well. Sporting clubs, workplaces etc. We believe it would seem a bit hypocritical if we were trying to get them to implement policy’s/programs, but we weren’t doing the same ourselves, so we have an informal policy that anything we do in the community that is also relevant to the Hospital, we will implement within our own organisation too. Not prohibitionAs I said earlier, because we are conducting many community based projects, we are working with a number of organsiations to assist them in becoming healthier eg. Sporting clubs, workplaces etc. We believe it would seem a bit hypocritical if we were trying to get them to implement policy’s/programs, but we weren’t doing the same ourselves, so we have an informal policy that anything we do in the community that is also relevant to the Hospital, we will implement within our own organisation too. Proactive ED Charge Nurse Gone from 3 to 30 referral list. Barrier – shortage of nurses, therefore time to implement activities above and beyond their patient care duties.     15. Where to from here? Implement recommendations from Staff Health and Wellbeing Survey Inclusion of Health Promotion plan in all Division’s Business Plans. Increased collaboration with Clinical Services to implement more initiatives for patients, family and friends. Reorient our health service and change the official name of the organisation. Change the image of the Hospital from an organisation that is seen to only care for the sick, to one that utilises a preventative and health promoting approach.
