How did we make the plan? • By sharing information in the Planning and Checking Group and listening to peoples’ experiences of life in the Swale community • Discussing the issues raised and how we could try to make things better for people with a learning disability and their families • Working in partnership with other organisations to make sure changes happen in Swale

  7. We are keen to make sure that • We build links with local special schools and colleges to raise awareness of Valuing People Now, Person Centred Planning and the District Partnership Group

  8. We will make sure this happens • By attending ‘Moving On’ days at schools and colleges • Making contact with younger people going through transition and their families • By working with the Swale Transitional Care Manager to look at holding a Junior DPG
