  7. Equity Value Statement • Equity – A healthier community for all members can be achieved by understanding the strengths and needs of diverse populations, and by addressing with dedication the underlying social determinants that contribute to inequities in health outcomes.

  9. Linking Health Equity & MAPP MAPP engages the community around health. MAPP provides a strategic framework to formulate goals and strategies that address the root causes of health inequities, to achieve the highest level of health for all people. The MAPP process can increase community awareness and understanding of the impacts of social determinants on health outcomes. 10

  10. Linking Health Equity & MAPP The Coalition embodies the organizations, entities, and individuals who together can create social and physical environments that promote good health for all. Transforming how people think about health can occur at any phase of the MAPP process. Coalition stakeholder resources and assets can be leveraged to address the unequal distribution of social determinants of health, and work towards greater health equity in our community. 11
