  3. Meeting Objectives • Agree on AIM for Collaborative and begin work on individual hospital AIM statements • Learn about interventions and begin thinking of how to implement • Increased awareness of other initiatives on readmissions

  4. Data Reports • Revised data reports • Which patients are readmitted • Payer • Location from which they were initially discharged to • Number of times readmitted • To same/different hospital • Why patient was readmitted • Financial impact

Other Readmission Initiatives • CMS QIO Program – Care Transitions • IHI/Commonwealth initiative (May) • Medicare • Calculating 30 day risk adjusted readmission rates for CHF, AMI and Pneumonia (July 1, 2007 through June 30, 2008 • Incentives to reduce avoidable readmissions • Direct adjustment to individual patient payments • Use of a performance based payment • Public reporting

  6. Other Readmission Initiatives • American Hospital Association • Elder Care Project –Panhandle • LIP- funded Readmission project with 11 county health departments • Others???
