There isalso good evidence for treatment in symptomaticvaricoceles, as well as in adolescents withtesticulargrowth retardation and/or abnormalsemenparameters. Althoughmost techniques have similarresults in terms of outcome, each has itsownadvantages and disadvantages, and itis up to the provider and the patient to determinewhatwillwork best for them. Furtherstudies,includingpredictivefactors for success, more objective ways to diagnose, as well as nontraditionalmeasures of success (e.g., decreased DNAF, improvedtesticularfunction) are needed to help us to betterunderstandthis condition.

Effect of varicocelerepair on sperm DNA fragmentation: a reviewM Roques et coll 2018 Abstract Varicocele, the leading cause of male infertility, can impair spermquality and fertility via variousoxidative stress mechanisms. An imbalancebetween excessive reactiveoxygenspecies production and antioxidant protection causes alterations in nuclear and mitochondrial sperm DNA, thusrendering a subset of varicocele men less fertile. In particular, spermDNAfragmentationisusuallyelevated in men withclinicalvaricocele in bothabnormal and normal semenparameters by thecurrent World HealthOrganizationcriteria. In thisreview, wediscuss the evidenceconcerning the association betweenvaricocele, oxidative stress, and SDF, and the possible mechanismsinvolved in infertility. Furthermore, wesummarizetherole of varicocelerepair as a means of alleviating SDF and improvingfertility. Lastly, wecriticallyappraise the evidencebasedalgorithmrecentlyissued by the Society for TranslationalMedicineaimed at guidingurologists on the use of SDF testing in men withvaricoceleseekingfertility. Currentevidencebased on carefulreview of publishedstudiesconfirms the effectiveness of varicocelectomy as a means of bothreducingoxidativelyinducedsperm DNA damage and potentiallyimprovingfertility. Varicocelerepairshouldbeoffered as part of treatment option for male partners of infertile couples presentingwith palpable varicoceles.
