Inspection Report Form 25D-100 • There are instructions with examples available on the construction forms webpage:
1.1 Project Name • Use the name shown on the plans • Should match the project name in the SWPPP and on the NOI Example Project
Use the Project Number shown on the plans and in the SWPPP • If there is more than one project number, use the first one unless directed otherwise 1.2 Project Number Example Project 12345
1.3 Project Location • Use City and State • If the project is not near a city, use Highway name and milepost number, for example “Parks Hwy, MP 45-55” Example Project 12345 Juneau, AK
1.4 NOI Tracking Number • Two different numbers • Found on the NOI Letters/emails from DEC • Usually do not change Example Project 12345 Juneau, AK AKR10AB12 AKR10CD34
Where to find your NOI Tracking Number: Example Letter from DEC • NOIs from DEC are posted for public viewing at the jobsite • Copies of both NOIs are kept in the SWPPP
1.5a Date of Inspection • Record the actual day, month and year the inspection took place Example Project 12345 Juneau, AK AKR10AB12 AKR10CD34 3/1/2017
If you are completing the form on a computer, save the document now. • The Project Number and Inspection date will now appear automatically in the footer of each page of the inspection. • If you are completing the inspection report by hand, the Project Number and Inspection date must be added by hand to the footer of each page.
1.5b Start/End Times • Record the hour and minute the inspection started and ended. Example Project 12345 Juneau, AK AKR10AB12 AKR10CD34 12:30pm-2:30pm 3/1/2017
1.6 Inspectors’ Names • Enter names for both the Contractor’s and DOT&PF’s Inspector. Example Project 12345 Juneau, AK AKR10AB12 AKR10CD34 12:30pm-2:30pm 3/1/2017 Pat Brown Morgan Smith

1.7 Inspectors’ Titles • Contractor’s Inspector’s Title: either Superintendent or SWPPP Manager. • DOT&PF’s Inspector’s Title: either Stormwater Inspector or Project Engineer. 1.9a & 1.9b AK-CESCL Certification Number and Expiration Date • Enter a current AK-CESCL or equivalent certification number and expiration date for both inspectors. 1.10 Describe Construction Activities • Briefly describe the current construction activity/activities since the last inspection. • Examples: clearing & grubbing, grading, culvert installation, paving, or pouring foundation. 2.1 Weather Since the Last Inspection • Check as many boxes as applicable.

Section 2.3 Weather at the time of this inspection • Check the box that best describes what the weather is during the inspection • Record the temperature
Section 3.0 Overall Site Issues • This section summarizes how the project is complying with various parts of the CGP. • It is likely easier to complete Section 5.0: Site-Specific BMPs first, and then return to this section.

Examples of documenting a reason for checking “No” for BMP Installed : Explain why in the comments If you check “No” and “No” !
These two typically have “No” and “No” checked, with no explanation needed.
Section 3.15 can also have “No” and “No” checked, or it can be left blank.
Section 3.0 Overall Site Issues (continued) • Other than the exceptions discussed before, the inspectors should be able to check “Yes” in the Response column to document permit compliance with these overall site issues. • See the Form 25D-100 written instructions (on the DOT&PF Construction Forms webpage) for examples for each category of Section 3.0. • This presentation will go into more detail on Corrective Actions when Section 5.0 is discussed.
Section 4.3 – Location of Discharge Points Circle by hand during the inspection • List the locations of all discharge points. • Must match the Site Map Hit the tab key in the last row to add as many rows as needed
Section 5.0 – Site-specific BMPs • The instructions below are on every inspection report and are a good resource. • See the Form 25D-100 written instructions (on the DOT&PF Construction Forms webpage) for additional examples.

Section 6.2 – Project Compliance • Examples of Non-compliance that must be documented in this section: • A 24-hour reportable discharge has occurred. • A deadline to initiate stabilization was missed. • A corrective action complete-by date was missed. • Required areas, BMPs, and/or discharge points were not inspected. • A required inspection was late or missed. • Attach additional pages if necessary.
The Certification Statement must first be signed and dated by the Contractor’s Superintendent and then by the DOT&PF Project Engineer.
