Tell us about your aim for improving frontline care? • How good do you want to be and by when? e.g. • We want to reduce avoidable pressure ulcers on out ward by…% by March 2011 • We want to reduce falls/CAUTI by…% • Improve Patients experience of care to >95% of patient willing to recommend the ward? • All of the above and more??

Baseline data • Please include your baseline pressure ulcer incidence/falls data/safety cross data/ patient experience scores if possible • Tell us your compliance with Risk assessment pu/falls • (to do this randomly select five patients case notes on your unit and check whether risk assessment was completed on admission) If 4 out of five were completed your compliance is 80%-3=60% 2=40% etc
