Source item elsewhere Yes No No No Yes Yes Practitioner to complete a Medical Practitioner’s Prescription (e.g., Mechanical Section Job Request 3) OR supply details which will be entered into a Medical Practitioner’s Prescription by responsible Technologist/Team Leader Review with Practitioner and clarify on Prescription Are the details clearly defined/ comprehensive? Consult Team Leader if appropriate to manufacture. Team Leader to carry out Risk Assessment if applicable. Has the Team Leader been consulted? No No Yes Yes Generate Rehab Quotation IF APPLICABLE TO CROSS CHARGE and agree costs/timescales Manufacture item and present to Practitioner for inspection Is manufactured item acceptable for use?
