  3. Web Referral Process – The Basics • Referring Person enters a referral via a secure link. • Immediately after submitting the referral, the referring person receives a confirmation email. 3. SJ referral coordinator receives an email notification that a referral has been received. A. Referral flows into the online referral program/holding area. 4. SJ referral coordinator contacts the patient within one business day and moves the referral into the kidney program.

  4. Web Referral Process – Key Facts • 95% of referrals received via web. • In the ten month period since implementation referrals are up 76%. • Saved 400 hours annually of staff time. • Tracking patients who do not answer on the first call is easier (create action).

  8. Physician and Dialysis Center Portal Basic Concept Allows the referring nephrologist access (view only or otherwise) to the transplant patient’s OTTR record. Only the physician portal currently exists.

Physician and Dialysis Center PortalThe Future • Allow physician groups to see all patients in the practice. • Up to 3 dialysis center staff to have access for patients from their facility. • “Landing page” that appears after login with a list of patients the user can access. • Possible addition of a “messaging” system that is not part of the OTTR record (unless added by a transplant staff member).
