Initial SNAC DATA Goals – 2007 Retreat • Demonstrate that the safety net system is real. • Look at what is currently available and identify what is missing. • ADDRESS what is missing. • Develop 3-5 uniform Data Elements. • Baseline survey of the safety net players. • Coordinate with other groups and/or initiatives that are undertaking complimentary work.

  4. 2007-2008 Accomplishments • Convened original Data group • Developed early list of measures combining the work of the Finance Committee • Undertook process of vetting measures for feasibility across the 4 participating sectors

  11. Combined SNAC and IAIMS Committees • Maintained SNAC/Clinic Representation • Gained Data, Research and IT expertise • GOALS: • Uniform Data to inform policymakers, providers, researchers and consumers • Safety Net Data Hub • EHR for safety net providers

  12. 2008-2009 Accomplishments • Convened Quality Sub-Committee and identified Quality Measures • Convened Access Sub-Committee and identified one measure • Developed Data Collection Capacity Survey • Piloted Survey across 4 sectors

  13. 2008-2009 – Continuing Work • Intern to provide TA to IRHF and CSC • Collect 4 data points for 2008 • Conduct Data Capacity Survey across all safety net providers – May-August 2009

  15. Next Steps toward realizing the HUB • Secure Funding to build and maintain Safety Net Data Hub • Build HUB Infrastructure • Formalize Data Sharing Agreements – as needed • Include addtl. Safety net data (hospital ED, Community Mental Health Programs and other)
