Care Home Capacity Tracker The Home Care Capacity Tracker allows care homes to upload details of available beds Helps to minimise delayed transfers Reduction of administration time required to locate available beds = reducing length of stays in hospital Discharge teams - make more informed choices when finding and selecting a care home.
Community Optometrists will be able to electronically refer patients for acute care, utilising secure NHS connectivity. Training is being provided by NHS Digital
Digital pathfinder Finding a button to open a door, can be harder than finding the door handle itself Signing in on a computer can cause more challenges than talking to a receptionist Focussing on people who are deaf, hard of hearing Increasing digital inclusion Utilise digital champions Co-design the smallest of things.
GP Online Consultation All GPs offer online consultations by April 2020 Secure online access to receive condition/symptom-specific advice, treatment, self-help guidance, submit admin requests Aims: • Deliver high quality care, enabling people to seamlessly navigate the service by ensuring people receive support based on clinical need • Signpostto appropriate services • Eliminatesphone / in clinic queues • Improves clinical efficiency = reduces clinician workload • Video consultations offered by 2021 • A single team implementing on behalf of the whole ICS to ensure consistency – better for everyone no matter where they live.
Joint Laboratory Information Management System A single LIMS across all 6 acute Trusts Hub and acute hospital modelof delivery Standardisation of investigations, reporting processes Enabling a view of patient reports across region Integrationto centrally capture “point of care” patient reports Consolidation of pathology services for specialist investigations.
National Pathology Exchange Implemented in all6 acute Trusts Eliminate expensive, unsafe paper-based lab to lab processes Improve data consistency = virtually eliminate transcription errors Reducethe process delay in the handling of paperwork, the batching of work for data entry at requesting and receiving laboratories Reduce amount of clinical, management time spent dealing with errors, ad hoc enquiries.
Scan4Safety Funding to implement in all 6 acute Trusts Provides NHS Trusts with tools to identify what care is administered to who, when, and by whom Releasing time to patient care Reducinginventory levels Increasing operational efficiency Potential to view inventory levels across the region.
Yorkshire Imaging Collaborative Provide joined up radiology service responsive to patient needs Serve 28 hospitals across 9 trusts, reaching 3 million people Programme of two parts; technology and transformation Technologyprogramme will deliver a common imaging platform across all services, currently being implemented Transformationprogramme will enable connectivity of all the YIC trusts, to allow for image, workflow sharing These will deliver on-demand availability of patient images, reports; reducing variation and standardising pathways.
