    • Employee Name
    • Submitted by: usually the name of the originator

check off every document listed once completed

    • Additional Notes: usually used to denote that TEMP. ALREADY HAS DOCUMENTS ON FILE
        • Do not SAVE any documents due to modifications.

  • You do not need to resubmit any of these documents

All forms can be found @ Human Resources - Recruitment

Please do not save any forms due to modifications

  • Be as specific as possible when stating the reason for revision.
  • ACCEPTABLE forms of revision should include either ALL signatures OR only the signatures of the Originator and Human Resources Personnel
  • When revisions are established please notify:
    • HR – Maggy Sivansay
    • Budget – Jackie Riffle
    • Payroll – Debra Gunter

Please include the word “REVISION” in the subject line of your email when sending notifications
