Program Enrollment Forms PAT HIPPY “Family Participation Agreement” (Doc in HUSA library) Program overview Home visits, group meetings, etc. Participation expectations Home visit work space, group meetings Communication with Home Visitor Understand commitment “HIPPY Application” Enrollment information • “Recruitment and Enrollment Record” (Curriculum fillable form) • Initial contact • Family logistics info • Names, ages, address, etc. • Child(ren) info • Medical basics • Family characteristics • High needs

  4. Program Participation Agreement 1. Program overview • Home visits: standard format, general content • Group connections/meetings 2. Participation expectations – Both ways! • Scheduled visits, meetings • Communication: Acceptable methods & timing

To ensure that program is effective and/or having desired outcome with families enrolled • To Whom?State & national office, funders • What info? Demographic, participation • How? Anonymously! Reports contain only numbers, no personally identifiable information • Exceptions?Mandatory reporting, as required by law

Program Participation Agreement • Draft/Sample language: As part of the program, I understand that certain information is shared with organizations or agencies that provide funding necessary for the program to operate. The data shared helps ensure that the program is effective. I understand that the information is shared anonymously and does not include any personally identifiable information about me or my family.

  9. Why We Collect Data • Demonstrate Program Effectiveness • Monitor Program Fidelity • Contribute to Broader Evidence-Base of HIPPY & PAT • Continuous Quality Improvement • Accountability to Existing Funders • Make the Case for New, Additional, or Continued Financial Support

  10. Why We Collect Certain Sensitive Data • Required Model Components AND ability to meet Individual Family Needs • For example: • Family Assessment Data • Intake Data • Screening Records • PVRs

Why We Collect Certain Sensitive Data • Funding Source Requirements; Two examples: • TGYS • Funding purpose relates to youth crime prevention and child abuse prevention • Must make case to state legislature annually • Show we are serving those that need it most • Need to be able to report aggregate numbers on certain demographics • MIECHV • Funding purpose relates to 6 key benchmark areas (maternal & child health; child injuries/child abuse & neglect/reduction in ER visits; school readiness & achievement; crime/domestic violence; family economic self-sufficiency; coordination & referrals), with 36 specific constructs • Must make ongoing case to state and the federal government • Must make case to Congress for reauthorization • Need to be able to track all of the information on each family served

  12. What We Do & Don’t Do with DataNote: We do not have access to individual data! What We Do What We Don’t Do Share Individual Level Data NEVER! WE DON’T EVEN HAVE ACCESS TO IT! Make judgments about sites based on data alone Share parsed data that is too small of a sample that it would violate confidentiality for a site Use data for commercial purposes • Report in aggregateto funders. • View data for a site. • To prepare for a site visit • To provide targeted T/TA • To advocate for specific funding   • Examine overall HIPPY & PAT trends and needs. • Evaluate overall HIPPY & PAT effectiveness. • In a given year • Over time • Within certain populations or program characteristics
