Ask a Public Health Nurse Program • Started 2012 • Frontline health professionals • Outreach to vulnerable & underserved populations • Collaboration with Cape Cod Hunger Network • Free health screenings & flu shots • Free access to professional nurse • Nurses consult with up to 30 clients/visit
BIOMETRIC SCREENINGS • Collaboration with Municipal Health Group • Bone density, blood pressure, glucose/cholesterol/HDL, hearing, vision, body mass index, skin cancer screening • Offered yearly to all towns BLOOD PRESSURE CLINICS • Located: • YMCA, • Chatham Community Center • Eastham Public Library • Falmouth Eats Together 14.1 Million adults are unaware they have high blood pressure (Million Hearts Program, CDC)
In Closing: Why Do We Need Nurses? • We bring professional public health nursing services to the residents • Provide mentorship and a site for nursing student interns – 29 students since 2011 • One dollar spent on immunizations is estimated to save $5 in treating preventable illness (ASTHO) • Future Growth: provide more point-of-care services in line with other county health dept.
