  2. REPORTING REQIREMENTSFOR FHWA-45, FHWA-810, AND FHWA-47 • FHWA-45: • Is required for all projects on the NHS = > $500,000. • Submit two weeks after the award of contract and no later than the fifth of the month after the end of the quarter. • Information on bid prices for major items

  3. REPORTING • FHWA-810: Bid tabulation • Is required for all F-A const. project on the NHS. • Submit to Division Administrator. • Certified by STA official. • Show details on 3 low bidders. • Submit within two weeks of the award of contract.

  4. REPORTING • FHWA-47: • Is required for all projects on the NHS that are $1 million or more. • Contractor completes at end of contract. • Information on final costs of wages, salaries and materials used by the contractor. • State DOT personnel verifies information.

  5. Office of Program Administration • UPDATE • Donald West retired on 8/3/04, to be replaced by Vince Barone in November. • The price trend historical data will be available on web site around March 2005. • FHWA Form-45/47 study is on hold.

  6. Office of Program Administration • UPDATE • The GAO conducted a review of FHWA Form-45 State construction cost. • Quality of the data was one big issue. • You may view the report at www.gao.gov , Report # is GAO-04-113R.

  7. Office of Program Administration UPDATE • The routing symbol is HIPA-10. • Margaret Hartman retired on 7/31/04. Send all forms to Claretta Duren till further notice. • Mr. Dwight A. Horne is the Office Director. • The office staff directory can be found on the fhwa website www.fhwa.dot.gov • Submit forms to FHWA, HIPA-10, Rm. 3134.

Office of Program Administration • GASB 34 data request: • If you need the historical data for price trends, send an e-mail to Ms. Duren; www.claretta.duren@fhwa.dot.gov Or fax to 202-366-3988 with your name, company name, and address. • Obtain software for imputing the data at www.fhwa.dot.gov/fedaid

USEFUL HINTS • Always report your project in a timely manner. • Make sure all information is completed on the forms. • Include your name and the State in the subject line of the e-mail. • Not reporting the data may cause your State to lose funding. • FHWA forms: www.fhwa.dot.gov/programadmin/contracts/ • The States are to use the new FMIS Improvement type codes, not the work type codes.
