  3. Our Mission and Aims To establish a comprehensive method of real-time data collection which is embedded into the stroke patient’s journey to enable accurate reporting and therefore improve measured performance and enhance patient care.

  4. Case for Change • Currently required to report retrospectively on IPMR, AQ, and SSNAP as well as local KPIs • Data collection & entry time consuming and detracts from clinical care • Single real-time data collection solution required for data collection at the point of care • Options: Spreadsheet, Kaizen, Cerner, commercial product .

  5. Links with Trust Values and Goals Data inaccurate and inconsistent Performance has deteriorated against national targets Financial and commissioning implications

  6. Our Approach Staff conversation –26/04/13 combined with planned Stroke Coordinators training event, including Stroke Doctors, Data Co-ordinator, Ward Manager SEG Data Group meetings to engage MDT

  7. Quick Wins Understanding of problem at Trust level Standardisation of data collection across sites

  8. 3 High Impact Actions • Visit to Newcastle to see live data collection system in action • CaptureStroke identified as preferred option, business case written and presented to EHIT and EPG • Funding identified and order placed

  9. Benefits to Patients/Staff/Trust • Accurate real-time data collection & analysis • Missing measures immediately identified • Awareness of current performance • Stroke nurses available for patient care • Staff morale/retention

  10. Measures of Success • Performance against regional/national targets • Patient satisfaction • Accurate data in public domain • Trust reputation • Potential for increased income

  11. Summary and Next Steps Order placed June 2013 Interface to be built summer 2013 Testing, training summer 2013 Implementation Autumn 2013 Without Listening into Action unlikely to have achieved current status within this timeframe
