Program Parameters • Applies to all fully underwritten TERM & UNIVERSAL life products • Ages 18-75 • Minimum Face Amount $250,000 • Maximum Face Amount $1,000,000 • GUL Survivor maximum face amount $2,000,000 • Non-Tobacco users • Base rating after normal credits to table 4 or less • Does not apply to “flat extra” ratings

  3. Program Parameters • If your client has several of the following characteristics, he/she may qualify for up to an additional two table credits from the base rating. • Any three characteristics = 1 Table Credit • Any five characteristics = 2 Table Credits • The best case final offer is Standard.

  4. Lifestyle Characteristics • Great Family History – No deaths from any disease prior to age 70 • Lifetime non-smoker • Preferred or better driving record • Minimal alcohol use (no more than 1-2 per day) and no history of alcohol abuse • Income >$100,000 or net worth > $1mil or a college degree

  5. Medical Characteristics • Optimal Blood Pressure control – treated or untreated(130/80) • Preferred or better build – Ages 18-60 • Standard Plus or better build – Ages 61-75 • Regular preventative medical care and compliant follow up • Cholesterol/HDL ratio <5.0 • Negative cardiac testing • GXT, non-imaged or images(stress echo, perfusion study), echocardiogram EBCT or angiography • GXT exercise performance >10 METS
