Progressive loss of responsiveness to stressors FRAILTY Co-financed by 22 MMSS & UE HP 2014-2020 2017 - 2019 22 MMSS Led by Health Ministry (Spain) ROBUSTNESS DISABILITY ADVANTAGE JA HARDLY REVERSIBLE REVERSIBLE Goal: Promote healthy aging through a frailty prevention approach: - developing a common understanding on frailty to be used in the EU, by policy makers and other stakeholders - supporting to the MS Progressive loss of intrinsic capacity Ferrucci et al., 2002; Gill, Gahbauer, Allore, & Han, 2006;Strandberg & Pitkala, 2007; Xue, 2011; Pahor et al., 2014, Rodriguez-Mañas & Fried., 2015, WHO, 2015

ADVANTAGE recommendations Ten domains: - Domain 1: Raising awareness, engaging stakeholders and empowering older people. - Domain 2: Commitment to action on frailty. - Domain 3: Promotion of healthy ageing and frailty prevention. - Domain 4: Early diagnosis of frailty. - Domain 5: Appropriate management of frailty. - Domain 6: Establish and continually improve an integrated model of care. - Domain 7: Education and training. - Domain 8: Research. - Domain 9: Implementation support (finance and ICTs) - Domain 10: Monitor quality and evaluating cost-effectiveness. Each domain includes a brief rationale, recommended actions, possible indicators and is illustrated by a case study example. Complete documents available at www.advantageja.eu

The Marche region – Reference Site • Number of inhabitants 1.531.753 • Number of ageing people 65-79: 248.662 • Number of ageing people 80+: 126.849 • Implementation of the Italian Roadmap of Advantage JA • Specific law addressing AHA (n.1 dated 28/1/2019), recognising the role of older people in the community and contrasting the phenomena of exclusion, prejudice and discrimination. • Strategic Regional HealthcarePlan 19-21 The plan was co-created with stakeholders from the professional and civil society and one of its main strategic objectives are: addressing frailty and chronicity from birth to ageing. • ProMis Network (the institutionalized Italian Network including all Italian regions • INRCA, the National Institute of Health and Science on Aging, in terms of sharing knowledge and good practice (e.g. Italia Longeva)

  6. Give visibility to the ADVANTAGE JA recommendations among health and care workforce, policy makers, managers and older people and their carers. Have civil society contributing to implement ADVANTAGE JA recommendations and have tangible results in everyday life, thus putting knowledge and evidence into practice. Raise awareness of the impact of frailty for individuals, families, communities and systems. Support people to share their stories about living with frailty. Help people from all sectors to understand what they can do to prevent and manage frailty. #FaceUpToFrailty – AIMS
