Ward 22General surgical ward – specialising in COLORECTAL surgeryTo deliver effective post operative care we liase and work closely with members of the MDT, in particular…Colorectal surgeons and their teamsEnhanced Recovery teamStoma CNS & teamColorectal CNS & teamPhysiotherapist & Occupational therapistsPain CNSDischarge Co-ordinator & Discharge Planning team

Collaborative Working with MDTSurgical teams(Ward rounds, Monthly SLM)Enhanced Recovery team (Ward meetings, Visits with new starters, Development of pathways, Monthly ERAS meetings, Monthly SLM)Stoma CNS & Colorectal CNS(Sessions with new starters, development of nursing care plans, Monthly ERAS meetings, Monthly SLM)

Collaborative Working with MDTPhysiotherapist & Occupational therapist(Daily handover with nurse in charge, working alongside nursing team, discharge planning)Pain CNS(Ward meetings, Visits with new starters, Monthly ERAS meetings)Discharge Co-ordinator & Discharge Planning team(Close working daily with nurse in charge, handovers, liasing with social workers and discharge planning team)

Post op… to dischargeIntroduction of new Surgical Departure Lounge (SDL)Positive impact on patients being discharged from an area set up to achieve this, which in turn facilitates post operative patients being transferred to the ward earlier in the day, which positively allows nursing staff to facilitate ERAS principles at an appropriate stage in patients recovery.Dedicated senior nurse responsible for discharge processOne area receiving and chasing all TTA’s Room available to carry out removal of drains, catheters etc and for privacy of discharge information.

Support Post Discharge (Significant concern line) Patient information card given on discharge regarding advice line Mobile phone held by nurse in charge of shift (Ward 22) Patient concern (triage) sheet for completion by Nurse In Charge Escalation to surgical team as required or relevant personnel eg. DN or HAH team
