  • She saw her doctor once the pain was bad enough to prevent her from performing normal activities

  • Her doctor diagnosed osteoporosis, and must consider the different types of treatments…

  • Dynamic bone deposition and resorption

  • Osteoblasts

    • Derived from stromal cells

    • Secrete organic bone matrix (osteoid)

    • Quantity tend to decrease as people age

  • Osteoclasts

    • Derived from macrophage cells

    • Remove mineralized matrix, break up organic matrix

    • Regulated by parathyroid hormone, calcitonin and several moleculessecreted by osteoblasts

      • These may all decrease with aging

  • Disruption in osteoblast and osteoclast activity leads to bone density problems

  • Osteopenia

    • Lower than normal peak BMD

    • Precursor to osteoporosis

  • Osteoporosis

    • Osteoclasts carve too deeply

    • Osteoblasts fail to fill enough bone back in

    • Fragility; vulnerability

Osteoporosis Treatments

  • Estrogen/Progesterone Hormone Replacement Therapy

    • Preventive; makes up for declining hormone concentrations in elderly

  • Raloxifene

    • Preventive; binds to estrogen receptors, mimics normal function

  • Bisphosphonates

    • Bind to calcium hydroxyapatite; inhibit osteoclastic activity

  • Parathyroid Hormone

    • Increases bone mass by preventing apoptosis of osteoblasts; also enhances formation of gap junctions

  • Osteoprotegerin

    • Inhibits formation of osteoclasts
