Electrosurgical Units Choosing the right electrosurgical unit is critical for a surgeon. Having the right equipment for the job will ensure a positive outcome and less complications for the patient. Choosing the right unit isn't always easy, but it's important to do your research and select a unit that will fit your needs. Saline Plasma Bipolar Saline Plasma Bipolar Compared to monopolar resection, bipolar resection is more effective in terms of coagulation and cutting power. It is less traumatic, and less bleeding occurs during the resection. However, there are minor risks involved. A study by Akgul and colleagues compared the effects of bipolar resection to those of monopolar resection. They found that there was a statistically significant difference in the depth of necrosis zone between the two electrode types. The mean difference was -0.0018 mm. The study was performed ex vivo after transvesical digital enucleation of the prostate. A randomization list was used to compare the two electrodes. Moreover, the study used reproducible methods and standardized procedures. The study included 157 patients. Both electrodes were of similar design, and were used for cutting and resection. The electrodes included a loop electrode and a button electrode. They also included a resection loop to assess necrosis depth. Coag mode Coag mode Depending on the nature of the procedure, an electrosurgical unit may be used to generate a variety of tissue effects. These include haemostasis, cutting, and fulguration. The degree of coagulation is determined by the strength and duration of the current waveform. Dispersive electrode Dispersive electrode During electrosurgical surgery, the dispersive electrode, also known as the grounding pad, serves as a path for returning current from the electrosurgical generator to the patient. This pad is located near the operative site and is positioned over a substantial muscle mass. It should also be positioned away from bony prominences and pressure points, and should avoid adipose tissue and hairy surfaces. Electrosurgical units produce high currents to coagulate and cut tissue. The higher the voltage, the higher the chances of arcing, and the higher the current density, the more likely there is to be an unintended burn.
The main risk to an electrosurgical unit is unintentional grounding. Good use of the dispersal electrode can reduce this risk. When a dispersive electrode is not used, it should be kept in a non-conductive plastic holster. A common type of scratch pad can scratch the electrode tip grooves, causing eschar to build up. However, lack of funding and poor infrastructure can make it difficult for many people to access safe surgical care. In Low and Middle Income Countries, there are six billion people without access to safe surgery. A project at Delft University of Technology (TU Delft) aims to produce an add-on module that can convert conventional dental electrosurgical unit into advanced bipolar power. The aim is to produce a reliable, accurate and efficient module that is compatible with the bipolar vessel sealing system that is already in use. This project uses simple, low cost and easily accessible components. It also includes an indicator circuit and alarm circuit.
