    Slide 1:Meeting the needs of Southeastern Ohio Seniors and their Caregivers through Community Based Long-Term Care Services

    Slide 2: What are Community Based Long-Term Care Services (CBLTCS)?

    Slide 3: They are a wide range of non-institutional services, including supportive, health and personal care, which help older people who need assistance maintain maximum independent functioning in their own homes or a substitute environment of their choice.

    Slide 4:Long-Term Care Consultation

    a free in-home assessment provided to anyone regardless of age or income. provides individuals with a guide to information and services to help them make informed decisions regarding long-term care.  presents choices and resources for meeting personal care needs or the needs of someone being cared for.

    Slide 5:INFORMATION & REFERRAL

    provides information about community resources (legal/financial, transportation, support groups, home health services, hospice services, health services) and emergency services (financial aid, shelters, food, clothing) and makes referrals to any program within or outside of our agency as appropriate.

    Slide 6:PASSPORT PROGRAM

    State and Federally funded with cost cap provides in-home alternatives to nursing home care for low-income seniors.  if eligible for PASSPORT, a case manager along with the consumer design a care plan to meet the person’s needs at home by arranging the most appropriate options of in-home services to supplement care provided by family members and friends.

    HOMEMAKER SERVICES PERSONAL CARE RESPITE ADULT DAY SERVICES MINOR HOME MODIFICATIONS HOME DELIVERED MEALS DIETICIAN SERVICES THERAPY SERVICES COUNSELING HOME MEDICAL EQUIPMENT TRANSPORTATION TO MEDICAL APPOINTMENTS

    Slide 8:Caregiver Advocacy Program

    addresses the growing needs of informal caregivers and those they care for Caregiver can be any age. Care recipient must be over 60 years old OR 60+ caregiver caring for a family member with MRDD who is under 18 years old. No financial eligibility requirements Level of care of recipient determines services

    Slide 9:The Caregiver Program includes the following:

    TELEPHONE SUPPORT GROUP offers the benefits of a support group to caregivers who are unable to leave their homes. Caregivers need to call the Area Agency on Aging for a one-time registration. MEDICAL EQUIPMENT PURCHASES to assist and compliment the care a caregiver provides, medical equipment purchases are limited and determined by the program coordinator on a need basis. 

    Slide 10:

    CAREGIVER TRAINING in-home training to caregivers. RESPITE CARE such as personal care, homemaking, or adult day services, provide short-term assistance for an older person so the caregiver can have some free time. LEGAL SERVICES provide education and assistance to caregivers in matters of Living Wills, Health Care Power of Attorney information and documentation, and Durable Power of Attorney information and documentation.

    Slide 11: PROJECT LIFESAVER a rapid response search and rescue program used to reduce the potential of harm to individuals suffering from Alzheimer's, Downs Syndrome, Autism, traumatic brain injuries, and cognitive impairments who may tend to wander away from their residences and become lost. The individual wears a one-ounce radio transmitter which emits a silent signal 24-hrs a day. The transmitter is housed in a wristband unit that looks like a watch. Once notified that the individual has wandered, the Sheriff’s Office responds to the wanderer’s area and starts searching with the mobile locator tracking system.

    Slide 12:LEGAL SERVICES

    are offered to assist in making arrangements for handling personal and financial affairs in the event that you would become unable to manage them for yourself. 

    Slide 13:Residential State Supplement (RSS)

    a program that assists older persons and qualified people with disabilities who do not medically qualify for nursing home placement, but who live in approved group living settings such as board and care homes and adult foster homes. Medicaid card

    Slide 14:Referrals for other services provided by the AAA or other agencies such as:

    Nutritional Services Alzheimer’s Respite Medical Escort

    Slide 15:Long Term Care Ombudsman Program and Ombudsman Volunteers

    advocates for quality care and addresses the concerns about the quality of long-term care services and negotiates for the solution of problems that arise between providers and consumers of long-term care services. volunteers are the first contact most consumers and families have with the program.  Regular visits by volunteers empower and educate residents regarding their rights while living in long-term care facilities.

    A federally funded program offering assistance for homeowners 60+ who need home modifications and repairs.   Income based assistance

    Consumer directed program Must meet PASSPORT eligibility Must be able to self-direct care and services Hire own workers and manage budget

    Slide 18:Assisted Living Waiver

    Alternative to nursing home placement Medicaid funded with cost cap For individuals 21 and older living in a NF or on a waiver program Personal care, homemaking, nutrition counseling, transportation services “tiers” of services Approved AL facility (Riverside Country Care Center)

    For medically eligible, Medicaid eligible under 60. Apply at County Department of Jobs and Family Services

    Be familiar with the services that are available in Southeastern Ohio Discuss with seniors and their caregivers how these services can enable seniors to reside in the setting of their choice while maintaining their autonomy and choice of services. Make a referral to the AAA or encourage consumers to call the AAA at 1-800-331-2644
