  2. What is personalised care? People having choice and control over decisions that affect their own health and wellbeing within a system that harnesses the expertise, capacity and potential of people, families and communities in delivering better outcomes and reducing health inequalities. This incorporates: . • a proactive and universal offer of support to people with long term physical and mental health conditions to build knowledge, skills and confidence through supported self-care and community-centred approaches whole population approaches to supporting people of all ages, and their carers, to manage their physical and mental health and wellbeing and make informed choices and decisions when their health changes Intensive approaches to empowering people with more complex needs to have greater choice and control over the care they receive

  3. “I can plan my care with people who work together to understand me and my carer(s), allow me control, and bring together services to achieve the outcomes important to me.”

Some of our successes so far Maternity Pioneers Patient Activation IPC 28,040 as of Mar 2018 173,143 people covered 70,000 assessments in 17/18 Nearly 19,000 PMCBs by March 2018 22% increase on previous quarter 70k+ have personal care and support plan 16,000 attended formal self-management programmes 83% of CCGs completed choice self assessment PHBs

Examples of progress Across Continuing Health Care, direct savings of c.17%; indirect savings of £4k per person per year 77% of people would recommend PHBs to others with similar needs 75% of people believe their health and wellbeing to have improved as a result of having a PHB WARRINGTON STOCKTON-ON-TEES NOTTINGHAMSHIRE FYLDE COAST Personal health budgets in end of life care - 83% were able to die in a place of their choosing, against an average of 26% 85% improved or maintained level of patient activation (average shift of 9.7 points), with attendant decrease in likelihood of hospital admission Significant reduction in unplanned hospital admissions for older people with frailty £19,000 saving in transport costs for siblings with very complex health conditions 15% reduction in delayed transfers of care from hospital Lease their own adapted vehicle through a personal health budget for journeys to day centre and respite, instead of a commissioned transport package One week’s worth of traditional services funds six weeks of services commissioned through a personal health budget 10% reduction in year on year A&E attendances
