  6. Stan/ Eval Lt Col Leslie Vazquez – Advisor Electronic CAPF 5 • The current method of processing a CAPF 5 check-ride involves using a paper-based checklist. The paper trail starts with the check ride applicant presenting the Form 5 document to the check pilot. The check pilot completes the form at the end of the check ride and hands it back to the applicant. The form is then passed through the applicant’s wing process for validation.

Stan/ Eval Lt Col Leslie Vazquez – Advisor Electronic CAPF 5 • With the advent of technology and the mandate to move to a paperless system by the National Board, it makes since to change the current paper form to an online format to be initiated, completed, and submitted by the check pilot. The electronic form will be time saving and provide additional trend analysis capabilities to meet our Statement of Work responsibilities.

Stan/ Eval Lt Col Leslie Vazquez – Advisor Electronic CAPF 5 • Other than substituting an electronic form for the paper form, the check-ride process remains the same. • Completed copies of the document can be downloaded and printed, as necessary. • The online form will contain all of the current elements found on the current Form-5 and trend analysis information will be automatically submitted in real time.

FEMA Col James Rushing – Coordinator Ardent Sentry • On December 16, 1811, the New Madrid earthquake struck an area where Missouri, Arkansas, Tennessee Kentucky and Illinois are in close proximity. It is believe to have been a level 9 and felt in area of 50,000 square miles. If a major earthquake of that intensity were to occur on the New Madrid fault today, it could be the most devastating natural disaster the Country may ever experience. A number of state and federal agencies work together in preparation.

FEMA Col James Rushing – Coordinator Ardent Sentry • New Madrid Earthquake exercise 16-20 May 2011 • Participation in this exercise will include • 4 Regions – SER, SWR, NCR and GLR • 6 Wings – TN, KY, IN, IL, MO, and AK • An Area Command will be established in Little Rock, AK • FEMA Regions include 4,5,6 and 7
