Disease Prevention/Health Promotion • Service Implementation: • Other information to give a good picture of the service you are implementing: • Who is overseeing (AAA staff, contractor) • Locations where programs are held (meal sites, community centers, senior centers, churches, schools, etc.) • Partners, other community organizations • Any other descriptive information
