  2. New Functionality Wiz/HEO Provider will have ability to: • Hold an active med on a specific date/time • Resume a med currently on hold by • Date/time • Number of Day(s) • Number of Dose(s)

Problem We Are Trying to Solve • Home or Maintenance meds are not resumed • Adverse Events • Examples: • Antibiotics not restarted after therapeutic levels reviewed • Psychotropics not restarted • Transplant meds • Pt admitted to ICU had home antipsychotics discontinued for evaluation.  After extubation transferred to general care unit. • Pt had active delirium and required transfer back to ICU to manage withdrawal and redosing of home antipsychotic.

Scenario 1: HOLD by Provider HOLD indefinitely Requires a current med order Link for hold when selecting med Provider selects when to start hold • Date or • NOW Provider selects when to resume by • Date/time • Number of days, hours or doses after placed on hold • Select ENTER for unlimited hold

  5. Scenario 1: HOLD by Provider WIZ/HEO, OPC, eMAR displays Held Care Organizer • Held appears Status column • Nurse to reconfirm med HED • No schedules will appear in Admin Rx

  8. Future Hold and Care Organizer • Med placed on hold in future • Care Organizer will display Ordered not Held • Will change to held on the day to be held

Scenario 1: HOLD and Resume by date Provider orders: Lisinopril 15mg po q1h starting at 1300 Team decides to place med on hold by doses starting Nov 15th 1800 through Nov 16th 10:00am Provider selects hold to begin • 11/15 1800 Provider selects resume • Nov 16 1000 • The 10:00am WILL BE A SCHEDULE TO GIVE
