  1. Health Center Health Center Integrated Rapid HIV Testing Dental Model Front Desk Front desk registers patient (new) or checks in on screen (existing). Encounter form printed and, together with ROUTINE SCREENING FLOW SHEET, placed in bin for pick-up by dental hygienist or tech. Waiting Room • Dental Hygienist or Tech escorts patient to dental operatory • During intake Hygienist/Tech offers RAPID HIV SCREENING • If accepted, patient signs consent (opt-in states only) • Sample obtained and transported to lab for processing • Dentist performs exam; orders necessary follow-up visits and/or referrals. • Hygienist/Tech enters results on Routine Screening Flow Sheet • “Negative” results given by Hygienist or Dentist; reactive results given by dentist • Patient discharged; Hygienist/Tech places the Routine Screening Flow Sheet in project folder Dental Operatory Laboratory If rapid HIV test is “reactive”; Clinical Coordinator from the primary care center is called to escort patient to the primary care center for a blood draw. Patient takes encounter form and proceeds to check out area for scheduling of next visit. Co-payment collected, if appropriate. Check Out Created by NACHC for use in health centers, April 2010
