Bei der Aufnahme wird gemeinsam mit BW und Angehörige eine medizinische, biografische und pflegerische Anamnese durchgeführt. Resultiert aus der Anamnese ein Schmerzproblem? Nein Das Verhalten des BW wird fortlaufend auf potentielle Schmerzen hin beobachtet. Ja Nein Deutet das Verhalten des BW auf Schmerzen hin? Ja Die Schmerzersteinschätzung wird durchgeführt. Kontinuierliche Schmerz-erfassung wird begonnen. Ärztliche Anordnung für Medikation einholen und unverzüglich umsetzen. Verordnung von Medikamenten und Therapien Nicht-medikamentöse Therapie wird geplant und umgesetzt. Kontinuierliche Schmerz-erfassung wird fortgesetzt. Reduktion der Schmerzintensität auf NRS 3/10 Medikamentöse und/oder nicht-medikamentöse Therapie werden angepasst. Nein Ja Kontinuierliche Schmerz-erfassung wird fortgesetzt.
