OUTCOMES Clinical 92% of Patients Rate the care they received as Good or Great All sites have achieved Level I NCQA Medical Home Participation in state collaborative on diabetes and preventive health care Average HbA1c in Network is 8.0 Patients who complete dental care plan- 51% Children fully immunized by age 24 mos-80% Financial Budget growth: $600,000 in 1991, 15 million in 2010 Surplus in 18 out of 19 years Received four expansion grants Serving more than 17,000 patients in 2010
