  2. AIM STATEMENT(PLAN) Increase Utilization of MAT

  3. NEXT STEPS(ACT) Provide consistent and ongoing communication to patients AND family members regarding MAT information. Added brochures in family program area. On site education programs weekly for all patients regarding MAT with focus on Vivitrol.

  4. RESULTS(STUDY) Bowling Green yielded 3% more MAT enrollments in Q2 than Q1, 2012. *Q3-2012 yielded 43% less than Q2-2012. Q4 is currently at 3.5% more than Q2-2012 * Turnover in PA-C

  5. NEXT STEPS(ACT) • Continue to provide environment where patient education occurs on a consistent basis. • Assure processes are in place for MAT enrollments regardless of any staff turnover

  6. IMPACT( Business Case/Lesson Learned) • More information readily available the better. • Relying only on passive enrollments is not best route. • Multiple ways of distributing information most effective. • Must ensure processes are in place by “PROCESS” – not “PEOPLE” (example: PA-C turnover)
