  2. Initial screen after log-in. Select applicable Endorsement folder.

  3. Downloading Evidence from the original report (brings up file download box)

  4. Originator Information concerning a HAZREP/SIR requiring endorsement appears.

  5. Causal Factors (CFs) concerning HAZREP/SIR requiring endorsement appear.

  6. + Click here to open a Causal Factor (CF). Click to add a CF not already in HAZREP/SIR.

  7. ENDORSEMENTS Causal Factor Analysis will appear.

  8. ENDORSEMENTS Endorsement Causal Factor (CF) Options: Concur, Do Not Concur, Restate, or Originating for “Newly” added CF.

  9. Justification If an endorser makes any changes or additions to any causal factor conclusion, RAC, or recommendation he/she will do so and provide detailed analysis as to why the changes were made. The endorser is afforded the opportunity to add, restate, or delete any portion of the previous endorsement

  10. + Click here to open a Recommendation (Rec). Click to add a Rec not already in HAZREP/SIR.

  11. Recommendation Information will appear.

  12. Endorsement Recommendation (Rec) Options: Concur, Do Not Concur, Restate, or Originating for “Newly” added Rec.

The endorser is afforded the opportunity to add, restate, or delete any portion of the previous endorsement.

  14. Endorser’s Final Comments.

  15. At this point, the Endorsing Chain should have already been established by the Controlling Custodian. However, in certain circumstances an Endorser may have to “add” an additional endorser IAW the OPNAV 3750.6 (series) Instruction.

  16. Page will display all validation errors requiring attention prior to submittal.

  17. Ability to preview document prior to submittal. Save prior.

  19. Reconvene Option Available.Any endorser who finds an investigation incomplete, or SIR inadequate, may reconvene the AMB and direct them to address the specific areas of concern. This must be done via WAMHRS infoing all concerned. The endorser who reconvenes an AMB must detail why he/she is reconvening the board and provide specific guidance as to what the AMB should address.

  22. Use the Endorse Report to release the report once complete.

  23. This justification box will pop up. Enter your justification if none then type “None”
