  4. Tool = Process Index (customized to your HSIP)

  5. Process Index • Step 1—Periodically assess progress • Step 2—Focus--identify supports and barriers • Step 3—Adjust and determine what to do • Step 4—Act on plan

  6. Achieve Periodically Assess Progress HSIP Goals and Objectives Focus on Planned Activities Act Adjust as needed

  7. Process Index—Step 1 • Periodically assess progress toward completing activities under each objective • Not Started • In Planning • In progress • In Place • How often? Every six months

  8. PROCESS INDEX—Name of District/Corporation *The ENTIRE process index, Steps 1-4, is completed at the end of each six-month assessment period.

  11. Track global progress toward fully implementing your Healthy School Improvement Plan (HSIP) • “Quick report” mechanism for others • District or corporation school health council, • administrative team and/or • school board.

  13. How to Complete • Count number of indicators are under each HSIP objective and make denominator • Example: Objective 10—Coordination of School Health Programs has 9 indicators, therefore denominator= /9 Example Summary Process Index

  14. How to Complete—Next • Every six months, count up all indicators “in place” under the objective and enter as numerator • Example: at 6 months 4 indicators are “in place”, therefore numerator = 4/ Example Summary Process Index

  15. How to CompleteFinal --Repeat for all HSIP objectives--

  16. Tracking Progress • Add up the number of indicators “in place” at a monitoring point for all objectives • Enter as numerator under Total • “Total” denominator is the sum of all indicators for all objectives e.g., 7+5+5+6+5+6+7+6+5+9+6 = 67

  18. Before Noon • Prepare (denominators) and complete (numerators) Summary Process Index for past6 months • Complete Process Index Step 3 for • Coordination • Policy and Planning • Adjust HSIP as needed • Prepare calendar of team meeting dates and times and site visit
