  1. ICSI Many a times when conventional IVFfails a couple is advised by the IVF specialist to undergo ICSI. The next question comes is what is ICSI and how it can help when conventional IVF failed. ICSI is a Micro Manipulation Technique which is the latest in IVF technology. The full form of ICSI is Intra Cytoplasmic Sperm Injection. In this process, sperm is injected into an egg and placed in the petri dish for incubation. Within 3-6 days when the embryo develops it is transferred into the woman’s womb to carry for nine months. Although it sounds simple, preparation for ICSI starts about 12-14 days before the procedure. Once the couple gets ready for ICSI, fertility drugs are given to the female partner to enhance fertility. The follicles of her ovaries will be regularly monitored by ultrasonography to assess. To trigger ovulation, 36-40hours before egg collection, hormone injection is given. During ovulation, eggs will be collected from the ovary by ultra sound guidance. At that time the male partner would be required to give sperm for fertilisation. In some situations donor or frozen sperm is also used. Frozen sperm should be thawed in advance. After egg collection, the egg will be injected with a sperm by glass needle. The concerned embryologist will choose the best sperm with good motility and inject into the egg. The rest of the process is same as conventional IVF. With the help of the incubator the embryos are kept for 3-6 days to grow. Once it grows, it will be transferred into the female partner’s uterus with the help of a catheter. Whether the female is pregnant or not gets confirmed only after 14 days by a blood test. Now we need to understand in what type of situation a couple is advised to try ICSI. It is generally done when severe male infertility prevents from successful pregnancy. The general conditions for ICSI are the following --- Low sperm count Physical blockage preventing the sperm to come out The movement or motility of the sperm is poor The morphology or shape of the sperm is poor A person after Vasectomy might have the desire Failed IVF cycles using conventional IVF When sperm has been retrieved surgically from the epididymis (MESA/PESA) or testes (TESA/TESE), from urine or following electro ejaculation When there are high levels of antibodies in the semen ICSI has a very good success rate and is widely used these days by IVF specialists. In this context we must know the risks of ICSI to get an idea. The risk of ICSI is actually a risk of IVF itself in increased chance of multiple births versus natural conception. By limiting the number of embryos transferred into the uterus, this risk can be reduced. The risk of birth defects in children conceived through IVF/ICSI is almost same as that conceived naturally.
