  4. Case Plans • The curser will be in the “DOC #” box: • Press F7 to clear out the old number • Type in the Offender’s DOC # • Press F8 to bring up the new number • You can also click on the yellow searchlight to perform these functions

  7. Case Plans • In order to have a good case plan,it is necessary to have: • THE NEED • Some examples of a Need would be: • “Substance Abuse”, “Cognitive Behavior” or “Education” • Needs end in “00”

  8. Case Plans • The Needs of offenders are determined at LARC using instruments such as the LSI or TABE test. • Needs are broad in scope.

  9. Case Plans • THE PLAN OF ACTION • The Plan of Action determines the correct Program to meet that Need. An associated program for: • “Substance Abuse” might be Regimented Treatment Program or Lifeline

  10. Case Plans ·“Cognitive Behavior” would be Thinking For A Changeor MRT • “Education” could be Literacy, ABE, or GED depending on how the offender scored on his/her TABE test.

  11. Case Plans • Now we will enter a case plan

  12. Case Plans • There are two steps to entering a case plan: • Enter the Need • Enter its matching Program • (So, if you have three Needs with a Program each, you will make six entries.) Say What?

  13. Case Plans • TO ENTER THE “NEED” • Click in “Type” box • Enter the Need: • Use the drop arrow to select • Or type in the 4-digit number if you know it • In “Effective Date,” enter today’s date • In “Review Date,” enter today’s date

  15. Case Plans How to Save a Key Stroke: When entering dates, you do NOT need to type in the dashes – OMS will automatically insert them.

  16. Case Plan • TO ENTER THE “PLAN OF ACTION” • Click in the “Type” box • Choose TFC, MRT, SAT, GED, etc.

  17. Plan of Action The PLAN OF ACTION has the first 2 numbers the same as the NEED

  18. Case Plans • Click on “Additional Info” section. Comments can be added, such as “On Waiting List” or “Has too much time remaining to enter program,” etc. (This will be to your benefit!)

  19. Case Plans • Look down the page and click in “Approval” box • Using the drop arrow, select the reason, such as “Pending”, “Entered” or “Completed.” If completed, enter the date of completion • Save

  20. Case Plans If an inmate has no assessed needs or has completed all programs, it still has to be entered as such (0000).

  21. Case Plans • THE MATCH GAME • The first two digits of the Program must match the first two digits of the Need (i.e. Substance Abuse has the programs offered with the same names for High and Moderate)

  22. Case Plans • Examples • The Effective Date on the Program must be the same or LATER than the date on the NEED, or it will NOT print out on the Case Plan and/or Adjustment Review!

  23. Updating Case Plans • When an inmate starts a program, change PENDING to ENTERED. • If the inmate is dropped from or fails the program, change ENTERED to the correct reason for being dropped or why they failed.

  24. Updating Case Plans • When an inmate completes a program change ENTERED to COMPLETED (code 50) and add the date. • When an inmate completes a GED it is code 51.
