  1. Injury Reporting LIFE THREATENING INJURIES CALL 911 NON-LIFE-THREATENING INJURIES Employees: • Notify the injured person’s supervisor of the incident. (If the supervisor is not available notify Harper Police.) • Escort the injured person to Health and Psychological Services (HPS). (During their open hours: Monday-Thursday 8am-6pm, Friday 8am-4:30pm). Have someone call HPS x6268 before proceeding, to ensure they can assist with the injury. • Health and Psychological Services will evaluate the injured person and provide treatment or refer them to Alexian Brothers Corporate Health Services (ABCHS) at 361 West Golf Road, Schaumburg, IL. (also referred to as the Occupational Health Clinic) • After hours when Health and Psychological Services is closed, the supervisor or designated representative should take the injured person to St. Alexius Medical Center Emergency Room at 155 N. Barrington Rd., Hoffman Estates, IL. • If the incident was due to a work related event the supervisor should complete an Incident/Injury Report (link to word doc)and send it to Human Resources, with a copy to the Manager, Environmental Health and Safety (Sara Gibson). Students: • Escort the injured person to Health and Psychological Services (HPS). (During their open hours: Monday-Thursday 8am-6pm, Friday 8am-4:30pm). Have someone call HPS x6268 before proceeding, to ensure they can assist with the injury. • After HPS hours, students should notify their instructor and should be referred to their own physician for treatment. If the incident was due to a classroom related event, the instructor should complete an Incident/Injury Report (link to word doc)ndsend it to the Mgr. Env. Health & Safety (Sara Gibson).
