  1. WORK RELATED ACCIDENT, INJURY, AND ILLNESS REPORTING AND INVESTIGATION PROCEDURES FOR REPORTING WORK RELATED INCIDENTS AND INJURIES

PURPOSE AND SCOPE: • This guideline establishes the procedures for reporting and reviewing work related injuries, illness, and near misses. • It is designed to help reduce injuries by reducing unsafe or hazardous conditions and discouraging unsafe acts or practices. • It applies to all GLCC Visteon employees, contractors, and vendors. • The detailed procedures described in the following slides should be followed immediately after an accident and/or injury. Accident Reporting Procedures (Employee Actions) If you are injured at work: • Report the injury immediately to their supervisor and Visteon Security at (734)710-2100. • Security will contact Police, fire and/or EMS as needed. • Security will notify Visteon Environmental, Health and Safety (EHS) as well as Visteon Health and Welfare of medical emergency via phone as soon as possible Contact your supervisor and Visteon GLCC security office at (734)710-2100

  4. Accident Reporting Procedures (Supervisor Actions) When an employee you supervise is injured at work: • Instruct the employee to notify Security if capable. If the employee is unable to contact security, the supervisor should contact security for the employee at (734)710-2100. • Control the scene of any serious incidents to protect others from potential secondary injuries and assure that no tampering of evidence occurs. Once security is on scene they will control the site. • Respond and investigate all incidents that occur in the supervisors area of responsibility including injuries and near misses. • Download and fill out the required fields of the Incident Notification Form. • Direct and coordinate the development and implementation of corrective actions. • Submit the completed Incident Notification From the Corporate EHS (Outlook: JMORAITI) and Visteon Health and Welfare (Outlook: SMEDZGI).

  5. Accident/Incident Investigation Definitions ACCIDENT: The National Safety Council defines an accident as an undesired event that results in personal injury or property damage. INCIDENT: An incident is an unplanned, undesired event that adversely affects completion of a task. NEAR MISS: Near misses describe incidents where no property was damaged and no personal injury sustained, but where, given a slight shift in time or position, damage and/or injury easily could have occurred.

Accident/Incident Investigation When do you conduct an investigation? • All incidents, whether a near miss or an actual injury event should be investigated. • Near miss reporting and investigation allows an opportunity to identify and control before a more serious incident occurs. • Accident/incident investigations are a tool for uncovering hazards that either were missed earlier or have slipped out of the controls planned for them. The objective is to identify root causes.

Accident/Incident Investigation Who should investigate? • The preliminary investigation of all incidents should be done by the supervisor in charge of the involved area and/or activity. • The safety department or the person in charge of safety and health should participate in the investigation and/or review the investigative findings and recommendations.

Accident/Incident Investigation The Investigative Report • Six key questions should be answered; who, what, when, where, why, and how. • Fact should be distinguished from opinion. • The report should include interviews with everyone with any knowledge of the incident. • A good investigation will likely reveal several contributing factors, and it will probably recommend several preventative actions.

Accident/Incident Investigation Implications of accident investigations • The primary purpose of accident investigations is to prevent future occurrences. • Information obtained through the investigation should be used to update and revise the inventory of hazards for hazard prevention and control. • Implications from the root causes of the accident need to be analyzed for their impact on all other operations and procedures.

  10. Accident/Incident Investigation Summary • Employee reports injury to supervisor and Visteon Security on date of injury or next business day. • The Supervisor shall secure the scene of a serious incident to ensure potential secondary incident do not occur until security arrives on the scene. • Supervisor makes an investigative report of the incident, enlisting the assistance of the safety department.
