State of Oklahoma Office of Management and Enterprise Services What information is reported on the 1095-B? • Name and TIN (or DOB) of every individual covered under the plan • DOB may be used only after reporting entity asks for a TIN 3 times • Employer information for employer sponsored coverage • Coverage provider information (health plan) • Month(s) of coverage for each covered individual • One-day rule: if you are enrolled in MEC for one day of a month you have MEC for that month

State of Oklahoma Office of Management and Enterprise Services What information is reported on the 1095-C? • Employee general information • Employer general information • Employee offer and coverage • Information on the health care coverage offered to full-time employees. This will be used by the IRS in determining an employee’s eligibility for the premium tax credit and to administer section 4980H (employer’s shared responsibility) • Indicator codes will be used to report certain information
