    5. Purpose Provide performance standards for local health departments. Engage and leverage national, state and local partnerships to build a stronger foundation. Promote continuous quality improvement. Strengthen science base for public health practice improvements.

    6. Phase I Local Public Health Performance Standards Assessment A partnership of local health departments and the CDC to develop clear, measurable standards for state and local public health systems. To ensure delivery and support of the ten essential public health services.

    15. Selection of Issue for Root Cause Analysis

    16. Phase II Leadership, Team Building and Root Cause Analysis (RCA) Training Enhance team building and leadership skills to improve overall workforce capacity of health departments regardless of size, structure or resources. Complete RCA for one selected issue in each health department, with written documentation that can be used to complete a performance improvement plan following training.

    17. Phase III Performance Improvement Program Plan Identify written performance objectives based on RCA. Create strategies to meet objectives. Identify key public health partners to collaborate. Design and implement plan.

    19. County Governance Survey To better understand perceptions of public health at the county level. To understand perceptions of county commissioners of local health issues.
