TRANSITION JARGON • OSEP-OFFICE OF SPECIAL EDUCATION • LEA- LOCAL EDUCATION AGENCY • PART B-PRESCHOOL SYSTEM • 100% TARGET INDICATOR-REQUIRED OSEP ACTIVITY • SPP-A STATE PERFORMANCE PLAN WITH 6-YEAR PROJECTION OF HOW WE WILL MEET TARGETS • APR-ANNUAL PERFORMANCE REPORT TO OSEP WITH DATA COLLECTED FROM YOU

  4. TRANSITION JARGON • TRANSITION PLAN—THIS IS THE FORMAL, WRITTEN PLAN DEVELOPED BY AEIS BY 27 MONTHS. THIS IS NEW!! • EI NOTIFICATION TO LEA-LETTER FORMALLY NOTIFYING LEA OF POTENTIALLY ELIGIBLE CHILD TO PART AND WHICH SEEKS TO CONVENE A MEETING NO LATER THAN 33 MONTHS. • TRANSITION PLANNING MEETING-MEETING WITH THE PARENT, EI AND LEA REPRESENTATIVE TO DISCUSS TRANSTION TO A LEA. • LEA REPRESENTATIVE —PERSON AT LEA DESIGNATED TO RECEIVE EI NOTIFICATION LETTER AND FOLLOW-UP AS NEEDED.

  5. TRANSTION JARGON • LEA REFERRAL MEETING —A MEETING SCHEDULED BY LEA FOR THE PURPOSE OF ACCEPTING THE REFERRAL. • PARENT REFERRAL —A PARENT MAY MAKE A REFERRAL THEMSELVES TO LEA UNDER CIRCUMSTANCES WE WILL DISCUSS.

TRANSITION JARGON • SUBSTANTIAL COMPLIANCE—FOR OSEP, STATES MUST SCORE AT LEAST 95-100% ANNUALLY ON ALL OF THE OSEP INDICATORS TO BE DETERMINED “IN SUBSTANTIAL COMPLIANCE” • TRANSTION TARGET INDICATOR #8 FOR EARLY INTERVENTION • TRANSITION TARGET INDICATOR #12 FOR PART B PRESCHOOL PROGRAMS ALABAMA IS CURRENTLY IN SUBSTANTIAL COMPLIANCE FOR EARLY INTERVENTION AND PART B PROGRAMS! CONGRATULATIONS!

TRANSITION TO DATE • 99% OF TIME, EI AND LEA COMBINE EI TRANSITIONPLANNING AND LEA REFERRAL MEETING. • ABOUT 10% OF FAMILIES OPT-OUT OF NOTIFICATION TO LEA-EI WILL DISCUSS NEW OPT-OUT POLICY WHEN PLANNING • EI WAS 98.7% COMPLIANT FOR TRANSTION PLANNING IN ’08 AND ABOUT 98% FOR ‘09. • PART B WAS 96% COMPLIANT FOR ’08 AND ALMOST 100% FOR ‘09

  8. TRANSITION PLANNING • WHY DO WE START PLANNING AT 27 MONTHS NOW (9 MONTHS IN ADVANCE-REALLY……..)? • INSURES TIME FOR SMOOTHER TRANSITION AND ATTENTION FAMILIES DESERVE • FAMILIES FEAR THE UNKNOWN • EFFECTIVE EARLY INTERVENTION NOVEMBER 3, 2009 • OTHER STATES BEEN ON BOARD FOR WHILE

TRANSITION PLANNING • AEIS SERVICE COORDINATOR DISCUSSES TRANSITION WITH FAMILY AND WRITES A PLAN BY 27 MONTHS • AEIS SERVICE COORDINATOR ADVISES FAMILY OF ALL PLACEMENT OPTIONS AT AGE 3 • AEIS PROVIDES FAMILY WITH OPT-OUT POLICY AND FORM IF THEY CHOOSE NOT TO HAVE THE LEA NOTIFIED ABOUT CHILD TURNING 3

  10. TRANSITION PLANNING • EI PLAN MUST INCLUDE WRITTEN STEPS FAMILY AND AEIS WILL TAKE TO INSURE SMOOTH TRANSITION • IF FAMILY CHOOSES TO HAVE LEA NOTIFIED, SERVICE COORDINATOR WILL SEND EI LETTER OF NOTIFICATION TO LEA WITH ADDITONAL INFORMATION IF FAMILY PROVIDES PERMISSION

TRANSITION PLANNING • FAMILY IS GIVEN OPPORTUNITY TO SIGN AN OPT-OUT FORM AT PLANNING OR WITHIN 10 DAYS • NO INFORMATION IS SENT TO LEA IF FAMILY OPTS-OUT OF NOTIFICATION TO LEA • IF FAMILY REMAINS UNCERTAIN DURING 10 DAY WINDOW AND DOES NOT RETURN OPT-OUT FORM, ONLY EI LETTER OF NOTIFICATION WILL BE SENT BUT NO ADDITIONAL INFORMATION (IFSP OR EVALUATIONS) MAY BE SENT TO THE LEA

  12. TRANSITION PLANNING • AEIS WILL ATTEMPT TO CONFIRM FAMILY INTENT DURING THE 10 DAY WINDOW TO CONSIDER OPT-OUT • IF FAMILY CONFIRMS THEY DO NOT WANT INFORMATION SENT TO LEA, AEIS WILL HONOR REQUEST AND ATTEMPT TO AGAIN OBTAIN OPT-OUT FORM • FAMILY WILL BE PROVIDED INFORMATION ABOUT HOW TO MAKE A PARENT REFERRAL IN FUTURE IF THEY CHANGE THEIR MINDS LATER--AEIS WILL NOT NOTIFY LEA PAST 30 MONTHS IF PARENT HAS BEEN UNDECIDED

  13. TRANSITION PLANNING • IF CHILD IS REFERRED AT 27-33 MONTHS, THE EI NOTIFICATION LETTER WILL BE SENT AS HAS IN PAST. • IF CHILD IS REFERRED TO AEIS AT 33 MONTHS (WITHIN 90 DAYS OF 3RD BIRTHDAY), AEIS MUST CONDUCT EVALUATIONS, WRITE IFSP AND PROVIDE SERVICES IF POSSIBLE • EI NOTIFICATION LETTER WILL NOT BE SENT IF CHILD REFERRED AT 33 MOTNHS • FAMILY WILL BE ASSISTED IN MAKING A PARENT REFERRAL TO LEA IF THEY CHOOSE

  14. TRANSITION PLANNING • IF CHILD IS REFERRED TO AEIS AT 34.5 MONTHS (WITHIN 45 DAYS OF 3RD BIRTHDAY, THE REFERRAL MAY BE ACCEPTED. AEIS WILL PROVIDE FAMILY WITH LEA CONTACT INFORMATION SO THEY MAY MAKE THEIR OWN PARENT REFERRAL. • EI NOTIFICATION LETTER WILL NOT BE SENT • AEIS IS UNABLE TO CONDUCT ELIGIBILITY, DEVELOP AN IFSP AND PROVIDE SERVICES WHEN CHILD IS WITHIN 45 DAYS OF 3RD BIRTHDAY • AEIS WILL INSURE REFERRALS MADE DIRECTLY TO THE STATE BY FAMILIES, AGENCIES AND PHYSICIANS THAT APPROPRIATE LEA CONTACT INFORMATION WILL BE PROVIDED

  15. TRANSITION PLANNING • EI PROGRAM SERVICE COORDINATOR MAY PROVIDE FAMILY DIRECTLY WITH LEA CONTACT INFORMATION. NO REFERRAL TO STATE OFFICE WOULD BE NECESSARY, REDUNDANT FOR FAMILY • THIS PROCESS IS SIGNIFICANT DEPARTURE FROM PAST PROCESS WHICH CAUSED CONFUSION FOR FAMILY • THIS NEW PROCEDURE HELPS FAMILY AVOID INCOVENIENCE OF DUPLICATE TIME AND EFFORT FOR EVALUATION AND PLANNING. LEA IS THE APPROPRIATE CONTACT POINT WITHIN 45 DAYS OF 3RD BIRTHDAY.
