Why Pathways? • Improve Clinical and Financial Outcomes • Improve Quality and Coordination of Care • Reduce Practice Variation, Increase Usage of Best Practices • Improve Communication and Information Transfer across Multi-disciplinary Teams and Organizations • Provide Consistent Patient Education across the Network

  4. What pieces do I need? • Pre-printed orders and protocols • Clinical Pathway • Teaching Checklist • Patient Pathway • Education Booklets.

  5. Where do I get Them? Grey Bruce Health Network Website http://www.gbhn.ca • All the GBHN Corporations have gone to print on demand • GBHN icon on desktop • How to print each piece of the package is located under Printing Instructions( single or double sided, paper size) • The individual units may choose to have prepared pathway packages including patient education pieces ready to use but must have process in place to ensure using most current materials

  6. Who starts Them • All patients admitted with a primary diagnosis that we have a pathway for should be on that pathway. • The pathway is to be started by the first unit that has contact with the patient and it is to be entered as an orderable under Cerner (ER, Pre-admit or Woman and Child Unit) • Start the pathway at the appropriate time frame If you receive a patient to your unit and the pathway has not been started and enter into Cerner

  7. What do I do with Them? • Have pathway materials you need ready with admission package when a patient is admitted • If you start the pathway in ER or pre-admit complete the documentation and forward the package to the receiving unit for completion of the pathway • Clinical Pathway remains as part of the patients chart and sent to medical records except for the Variance Form.

  8. Variance Record/Discharge Summary • Once the patient is Discharged the Variance Record/Discharge Summary form is sent to the Evidence-Based Care Program via inter-hospital mail pre-addressed envelopes are available for each unit and will be returned to you for reuse. (Envelope mailing system)
