Small business enterprise program (SBE) Goal Setting Methodology • Goals are established by City of Phoenix Goal Setting Committee • Successful Prime will work with the Small Business Engagement Team prior to goal setting • No established SBE goal for Design Phase • SBE goal for construction phase will be established upon substantial completion of design

Scope of work • Design Services Consultant • Investigate, identify and evaluate PCCP Transmission main rehabilitation options • Provide PCCP transmission main rehabilitation options, recommendations and complete rehabilitation design for 3.2 miles of 48” PCCP • Conduct alignment study to confirm PCCP main rehabilitation access feasibility along alignment • Coordinate with Street Transportation and other City Departments as required for options on right-of-way and/or City owned property • Coordinate with other design consultants on adjacent projects as needed • Conduct geotechnical evaluations to develop construction requirements • Investigate and identify existing utility and traffic control impacts and costs

Scope of work Construction Manager at Risk Services • Design Phase Services by the CMAR • Recommendations regarding design alternatives • Preliminary estimates for design alternatives • Provide detailed cost estimating, project planning and scheduling • Provide evaluation of all options, constructability studies and design • Assist in permitting processes • Cost models for selected design alternatives and value engineering • Participate with City in process to set SBE goals and participation • Protect owner’s sensitivity to quality, safety and environmental factors • Construction Phase Services by the CMAR • Construction of selected alternative per final plans and specifications • Select subcontractors / suppliers • Prepare Guaranteed Maximum Price (GMP) proposal (w/ approval by City) • Coordinate with various COP departments, other agencies, utility companies, etc. • Schedule and manage site operations • Bid / award / manage all construction related contracts • Meet City bid requirements including local and SBE participation goals • Bond and insure the construction

Submittal requirements • Submittals must be uploaded to the website by 12:00pm on September 27, 2019 • Submit only 1 copy in .pdf format • Maximum of 10 pages is permitted (information sheet, front and back covers, table of contents and dividers are not included in page count) • Do not use any symbols in title of documents Grounds for disqualification • Failure to submit electronically through the portal by the due date and time • Violating the “contact with city employee” policy
