  13. Indicator 1: Timely Services IFSP – Individualized Family Service Plan Timely*- All services must have been started within 45 days of service consent date for a child to be considered having timely services This indicator is addressed more fully in the Annual Performance Report (APR)

Indicator 2: Natural Settings IFSP – Individualized Family Service Plan; IDEA - The Individuals with Disabilities Education Act *Data based on infants and toddlers with IFSPs as of 12/1/2016 submitted under the IDEA Section 618 Child Count Data This indicator is addressed more fully in the Annual Performance Report (APR)

Indicator 3: Child Outcomes IFSP – Individualized Family Service Plan *Summary Statement 1: Of those children who entered the program below age expectations in each outcome, the percent who substantially increased their rate of growth by the time they turned three years of age or exited the program *Summary Statement 2: The percent of children who were functioning within age expectations in each outcome by the time they turned three years of age or exited the program This indicator is addressed more fully in the Annual Performance Report (APR)

Indicator 8: Early Childhood Transition IFSP – Individualized Family Service Plan *Timely: Notified (consistent with any opt-out adopted by the State) the State Educational Agency (SEA) and the Local Educational Agency (LEA) where the toddler resides at least 90 days and not more than 9 months prior to the toddler's third birthday for toddler potentially eligible for Part B preschool services This indicator is addressed more fully in the Annual Performance Report (APR)
