  3. Purpose Statement To provide a coordinated suicide prevention screening program to assess youth ages 11-18 in Ohio counties with the highest youth suicide rates and immediately link those at risk or in imminent danger to appropriate behavioral health care.

  5. Utilize and Enhance Existing Infrastructures EXISTING INFRASTRUCTURE Ohio’s TeenScreen Program • 13 Programs • 70 Individual Sites ENHANCE Offer Grants to Support: • Existing Evidence-based Youth Suicide Programs • Recruit New Programs in High Suicide Rate Counties

  7. Consent Goal The rate of active consents obtained will increase by 10% each year Year 1 Progress: 52% Average consent rate Year 1 (baseline) Year 2 Progress: Year 2 Goal: 57% 46%* Average consent rate Year 2 *One quarter of data remaining to be submitted

  8. Consents As a group we more than doubled the number of consents to be screened to youth and parents.

Mental Health Check Up Goal Number of youth receiving evidence-based mental health check ups increase by at least 50% Year 1 Progress: 2,833 of youth receiving mental health check ups (baseline) Year 2 Progress: 4,825* youth receiving mental health check ups Currently 85% of our goal achieved *One quarter of data remaining to be submitted
