Guy’s and St Thomas’ @Home (formerly Home Ward.Limited GP access currently, Pilot to be expanded to all GPs soon) Service Description:Admission avoidance and early supported discharge for acutely unwell patients, provided by an MDT. Service provides for:Immediate assessment for patients who are acutely unwell and have both medical and nursing needs would otherwise require a hospital admission e.g Exacerbation of COPD/ LVF, IV Antibiotics, UTIs, Acute pain management. Referral Route: Telephone referral to 020 30495751 (7 days a week, 08.00-23.00 hours) • Enhanced Rapid Response • Service Description:Home-based rehabilitation and care for adults whose function has rapidly deteriorated. • Service provides for:Patients who are medically stable but would otherwise require a hospital admission. E.g. fallers. • Referral Route:Telephone referral to 07917 554516 (7 days a week, 08.30- 18.00 hours) • Lambeth Admission Avoidance Teams – Lambeth GPs have access to all of the following: Psychiatry Advice Phone Line Service Description:Telephone access to Consultant Psychiatrists based within South London and Maudsley NHS Trust Community Older Adult Mental Health Services in Lambeth. Service provides for:GPs who would like advice when they have concerns about the mental health of a patient. Referral Route:Telephone Line – 020 3228 0300 HOT Clinics – Geriatrician led clinic Service Description:Same or next day outpatient rapid assessment by Geriatrician. Service provides for: Patients who are in urgent need of assessment by Geriatrician and MDT, but do not need A&E. Referral Route:Direct telephone lines @ KCH – 020 32996613 & GSTT – 020 71881465 Geriatrician Advice Phone Line Service Description:24/7 access to advice from Consultants in elderly care. Service provides for: Professionals needing advice regarding patients who have deteriorated and are at risk of admission.
