  3. Near collision, loss of separation, or air proximity

  4. Near collision – either positive or avoiding action or luck prevented a collision Air proximity – collision did not occur, but aircraft were close enough to cause alarm Loss of separation – loss of required separation; runway separation, required terrain separation (IFR below MSA), with IFR aircraft in controlled airspace

  5. Note: more dual hours flown per year – solo are around 60-70% of dual hours

  6. Safety Management Systems (SMS) • OMG! • Another acronym • Hospital pass • Relax

  7. SMS • Global trend where safety involved • ICAO requirement of all States • 141 ??? • NZ draft AC relates to CAR Parts119,139,145,172 • Extensive resources - don’t over complicate or do all at once • Complexity

An SMS is designed and implemented to: • Develop and improve the safety culture within the organisation • Manage risks within the organisation – with particular focus on risks which impact on safety • Provide for ongoing monitoring and assessment of safety performance • Make continual improvements

  9. The culture shift… • Move from intuitive environment to planned management of risk • Proactive approach • Identify hazards – prioritise • Some hazards might not become a risk

  10. CAA intentions include: • Possible roadshows • Provision of tools • Transition period • Share trend information • Safety promotion • Audit training

  11. Expectations • Philosophy - outcome • Process - principles • Practise – how to achieve • What do you want to achieve • How do you achieve it • How do you manage your system to provide for the outcome sought – competent, knowledgeable, skillful pilots, top attitude!

  14. Intuitive - no QS or SMS • Moans all day about sore head • Next time using “cat door” takes scab off last wound • Moans • Gets token sympathy • Repeats wound, ducks, uses another door • Sympathises with others similarly afflicted • Probably unproductive

  15. Quality System • Files report • Safety coordinator observes 9th report this month • Reports to CEO • Considers • No light over door • Door being damaged by hard heads • Recommends installation of larger door • CEO authorises job • No “cat door” • No sore heads • Productive staff

  16. SMS • Context of hazards established - hanger • Hazards identified – cat door • Risks analysed – light, damage, new installation • Risks evaluated – worth it? • Treat risk – new installation • Monitorandreview– unintended consequences • Communicate/consult principles

SMS • Context – fuel policy and procedures • Identify risk – no fuel policy/procedure on dipstick calibration/identification • Analyse – occurrences/instructor knowledge etc • Evaluate – consequence, probability, cost • Treat • CEO directs MC actions for next appropriate maintenance • Tanks drained, sticks calibrated & labelled “useable”/rego • MC develop fuel policy • Instructor training programmed on fuel policy • Communicate & review
